Forms allow you to leverage map tours to generate leads. This tool drives applications and enrollments, activity sign ups, proposal requests, and other business by actionable information of who is viewing your tour. Forms are completely customizable and can be added to any map tour.
Add Forms to a Map Tour with External Forms
This option allows the user to embed their own form.
1. Log into the Content Management System (CMS). Access this feature when creating a new tour or editing an existing tour. On the tour edit page change the setting Forms to yes to access the Form settings. Select form type External.
2. Enter form embed code. This can be from a CRM (Slate) or other form provider (Google forms). Here is an example of embed code that can be used with your form URL:

3. Form Timing - Select when you would like the Form window to display on the tour. There are three choices - the start of the tour, the end of the tour or a set number of stops into the tour.
**If using Slate Forms**
In order to embed a Slate form within your map tour, the form will need to be embedded on a standard webpage on your site first. Slate does not allow their forms to be directly embedded into third party platforms. You will also need to ensure the webpage can be embedded outside your domain.
Add Forms to a Map Tour with Custom Forms
This option uses the pre-built form in the map CMS.
1. Log into the Content Management System (CMS). Access this feature when creating a new tour or editing an existing tour. On the tour edit page change the setting Forms to yes to access the Form settings. Select form type Custom.
2. Fill out the information boxes for the Form. The video and description are optional, but recommended for optimal lead generation!
You can add multiple email addresses in the Notification Emails field by separating the emails by comma. Once a submission is received, each email will receive a notification with a link to view the submission as long as they have CMS access.
3. Select the fields for users to fill out in the Form. This is the step in which you can choose the type of data you want to capture, per your organization's restrictions and requirements. If you need additional guidance, ask your Client Success Manager.
- Custom Drop Down - This option will be multiple choice and can only be used once.
4. Form Timing - Select when you would like the Form window to display on your tour. There are three choices - the start of the tour, the end of the tour or a set number of stops into the tour.
