Reporting a Single Accessibility Issue
If you’ve noticed just one issue, no need to fill out a full template—thank you for speaking up!

What to Include
Please share as much of the following as you can:
- Page URL where the issue occurs
- Brief description of what went wrong and what you expected to happen instead
- Device, operating system, and browser
- Assistive technology used, if any (e.g., VoiceOver, JAWS)
- Screenshot or short video, if possible (strongly encouraged)

What to Include in the Template
General Information:
- Page URL(s) & date of testing
- Your contact information (in case we have questions)
- Device, OS, and browser details
- Assistive technology used (e.g., JAWS, VoiceOver)
- Testing tools used (e.g., axe, WAVE)
Issue Description:
- Where it happens (e.g., “Homepage search bar”)
- Severity (Critical, High, Moderate, Low, or Enhancement)
- Category (e.g., Screen Reader, Keyboard Navigation)
- What went wrong and what should happen instead
- Screenshots or code snippets (screenshots are strongly recommended)
